Health status and concomitant prescription of immunosuppressants are risk factors for hydroxychloroquine non-adherence in systemic lupus patients with prolonged inactive disease. Three studies have shown its benefit on survival in SLE patients 19-21. Because patients with severe renal insufficiency (serum creatinine clearance <60 ml/minute according to the Cockcroft‐Gault equation) were excluded from the PLUS study, we also analyzed blood HCQ concentrations in patients with chronic renal insufficiency who were followed up in 2 centers (Cochin and Pitié‐Salpêtrière hospitals in Paris), including 3 SLE patients who were receiving long‐term dialysis (included in a database of patients with renal insufficiency and blood HCQ measurements).
The authors concluded that the mean elimination half‐life of HCQ was 123 ± 45 hours. It is consistent with previous findings in 5 volunteers with RA who had received 6 mg/kg/day of HCQ for at least 6 months 36.
